Outreach Services

The aim of the Outreach service is to support you in retaining or learning new skills in order to remain living independently within your own home.
We can also give you the confidence to start enjoying yourself again with the reassurance that someone will always be there to support you.
Outreach will provide you with a tailored 1 to 1 support programme based upon your individual goals and aspirations. You can choose the types of support that you want and agree with your support worker how this is going to be achieved and delivered.
The service can offer a variety of support and activities including help in managing home affairs, carrying out daily routine activities within the home or enjoying favourite pastimes and hobbies.
This service is not a home help or domiciliary support service and therefore should not be purchased by customers requiring these activities to be entirely carried out for them.
